The London HART Hub is a low barrier recovery-oriented space supporting individuals experiencing homelessness with mental health and addiction needs. It provides access to evidence-informed, respectful, trauma-informed, culturally appropriate, and compassionate care. The Hub connects individuals to a continuum of care, including transitional supportive housing, safety, basic needs, healthcare, and mental health and addiction services.
The RN will utilize a holistic approach to client services, including assessment, treatment planning, and care implementation based on the bio-psychosocial model, following established policies and practices. Responsibilities include supporting treatment plans related to withdrawal management and addiction medicine, and providing ongoing support and safety strategies, including linkages to other addiction programs post-discharge.
