The London HART Hub is a low barrier recovery-oriented space that aims to support individuals experiencing homelessness with mental health and addiction needs to improve their health and wellbeing. HART Hubs will support its clients through access to high quality evidence-informed care that is respectful, trauma-informed, culturally appropriate, and compassionate. It connects individuals with a continuum of care tailored to their needs by providing access to transitional supportive housing spaces where individuals can access safety, basic needs, health care, and other mental health and addiction services under one location.
The Nurse Practitioner (NP) within the Hubs program will perform duties consistent with providing clients with integrated mental health and addiction-based medicine and primary care. The NP provides direct and indirect comprehensive patient care services in collaboration with various inter-professional teams across the organization, supporting mental health and addictions needs, including Withdrawal Management support.

WHAT WE'RE OFFERING:
- Starting salary: $65.69 per hour (Level 1 of the Nurse Practitioner Grid). Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8, $70.87 per hour. Internal applicants will be placed on the grid based on their seniority.
- Work schedule: 70 hours over a two-week period; Monday-Friday, 8 am-4 pm.
- Benefits: 9% in lieu of benefits.
- Pension plan: Immediate eligibility for optional enrollment in the Healthcare of Ontario Pension Plan (HOOPP).
HOW YOU WILL MAKE A DIFFERENCE:
- Provide services addressing both addiction-related and general medical needs.
- Offer non-urgent, health care services within the NP’s full scope of practice to clients facing barriers to primary health care, consulting with physicians for complex health conditions.
- Coordinate with Psychiatrists and other team members for comprehensive care.
- Maintain practice requirements including licensure, practice agreements, and malpractice insurance.
- Ensure confidentiality and act ethically, in line with CNO standards.
- Maintain clinical competence through evidence-based research and legislation.
- Exhibit excellent communication, decision-making, problem-solving, conflict management, and time management skills.
- Work effectively within an interdisciplinary team.
- Perform comprehensive care using advanced assessment, physical exams, therapeutic management, and referrals per the Nurse Practitioner Practice Standard (2018).
- Determine follow-up needs and assist in transferring care to primary health services.
- Support additional HUB sites as required.
Addiction Specific Competencies:
- Understand neurobiology of addiction.
- Conduct advanced assessments and interventions for clients with addiction issues.
- Manage patients with co-existing health conditions, including mental health, blood-borne diseases, and infections.
- Identify emotional, cognitive, behavioral states, and signs of substance abuse and withdrawal.
- Utilize evidence-based interventions, including medication management.
- Provide recovery-oriented, trauma-informed care addressing social determinants of health.
- Engage clients in strengths-based, resilience-promoting care.
- Educate individuals and families about addiction management.
- Collaborate across sectors and advocate for persons with addiction.
Health Care Services to Hub clients with no health care provider:
- Provide health services to clients aged 16+ meeting eligibility criteria.
- Lead development of expanded medical services.
- Assess and manage patients with addiction and co-existing health issues.
- Perform advanced assessments identifying emotional, behavioral, and health states.
- Use evidence-based interventions to meet diverse client needs.
- Offer recovery-oriented, trauma-informed care addressing social determinants.
- Collaborate with other professionals and sectors in providing care.
- Advocate for persons with addiction.
- Identify and seek new knowledge and skills in addiction and mental health care.
- Deal effectively with sensitive issues using a client-centered approach.
Outreach & Community Partnerships:
- Partner with community agencies and programs.
- Integrate counseling and support services.
- Coordinate with healthcare partners for community care plans.
- Assist in developing programs aligned with best practices and research.
- Promote integration of addiction medicine and primary care.
- Provide education and consultation as needed.
WHAT YOU'LL NEED TO APPLY:
Police Vulnerable Sector Check Requirements: Successful candidates must have a recent PVSC. Applicants residing in London should apply early due to processing times.
Qualifications:
- Current registration with the College of Nurses of Ontario as a Registered Nurse (Extended Class). Original credentials required during hiring.
- Completion of Prescribing Narcotics and Controlled Substances course.
- Current CPR certification.
- Valid driver’s license, insurance, and access to a vehicle.
- Current PVSC.
- Ability to attend work regularly.
Preferred qualifications:
- 2-3 years of community healthcare experience.
- Experience with marginalized populations, mental health, substance use, and trauma.
- Knowledge of community resources and barriers.
- Proficiency in various treatment and intervention strategies.
- Computer literacy.
- Master’s Degree.
- Suboxone online education.
- Experience in addiction medicine and harm reduction.
- Bilingual in both official languages (preferred).
