The London HART Hub is a low barrier recovery-oriented space supporting individuals experiencing homelessness with mental health and addiction needs, aiming to improve their health and wellbeing. The Hub provides access to high-quality, evidence-informed, respectful, trauma-informed, culturally appropriate, and compassionate care. It connects individuals to a continuum of care tailored to their needs, including transitional supportive housing where they can access safety, basic needs, healthcare, and other mental health and addiction services in one location.

The Registered Nurse (RN) within the Hubs program will utilize a holistic approach for client services, including assessment, treatment planning, and care implementation following the bio-psychosocial model, in accordance with established policies and practices.

The RN will support treatment plans related to withdrawal management, addiction medicine, and team-based care for clients engaging with the HART Hub programs. They will also provide support and strategies related to safety, ongoing support upon discharge, and linkages to other addiction programs such as RAAM, COAP, and treatment services.

WHAT WE'RE OFFERING:

  • Starting rate for this role is $32.13 per hour (Level 1 of the RN/OT Grid); successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8, $41.41 per hour. Internal applicants will be placed on the grid based on their seniority.
  • Work schedule: 70 hours over a two-week period; Monday to Friday - Days or Afternoon shifts.
  • Benefits: 9% in lieu of benefits.
  • Pension plan: Immediate eligibility for optional enrollment in the Healthcare of Ontario Pension Plan (HOOPP).

HOW YOU WILL MAKE A DIFFERENCE:

  • Screening, assessment, and provision of nursing interventions and supports to individuals referred to the HART Hub.
  • Collaborate with addiction and mental health service providers to support clients in crisis and needing supervised withdrawal management.
  • Support intakes, assess other health concerns, and lead addiction treatment plans.
  • Develop and maintain Medication Administration Records (MARs), manage/administer prescribed medications and controlled substances.
  • Promote HART Hub services to healthcare professionals, clients, and the community.
  • Conduct physical and psychiatric assessments, make appropriate referrals, and evaluate medication effectiveness.
  • Coordinate and administer depot injections in the community.
  • Assist clients in making informed health decisions.
  • Contribute to health promotion groups and identify high-risk clients for crisis intervention.
  • Perform emergency work calmly and efficiently, respecting client privacy and dignity.
  • Collaborate with clients and support teams to develop goals and care plans.
  • Act as a resource for staff and community partners, respecting client diversity.
  • Adhere to standards, complete documentation, and handle confidential information appropriately.
  • Assist with program development, implementation, and evaluation, and support other HUB sites as needed.

WHAT YOU'LL NEED TO APPLY:

  • Police Vulnerable Sector Check completed within the last six months.
  • Baccalaureate Degree or Diploma in Nursing (original or official transcript required).
  • Registered Nurse in good standing with the College of Nurses of Ontario.
  • Valid Driver’s License, insurance, and access to a vehicle.
  • First Aid/CPR certification.
  • Demonstrated ability to attend work regularly.

Preferred qualifications include:

  • ASSIST Training, Non-Violent Crisis Intervention Training.
  • Experience with electronic medical records, PS Suites.
  • Understanding of social determinants of health, neurobiology of addiction, and community resources.
  • Bilingual in both official languages preferred.
