Job summary

Alberta Health Services seeks a Registered Nurse to join the Self-Managed Care team within Integrated Home Care. You will provide comprehensive nursing care, develop client-centered care plans, and coordinate with families to adjust services as needs evolve.

You will support health promotion and holistic patient care, applying critical thinking and nursing processes. Provisional permits receive an hourly starting range, with pay adjusted upon full licensure and UNA guidelines.

• Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
• Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.

Using a person-centered approach, you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care.

In collaboration with the client/family you will ensure services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service authorization.

This position is with the Self-Managed Care Team which is a specialty Team within Integrated Home Care. This Team looks after the needs of our clients who choose to hire their own Caregivers.

Description

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values.

PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Required Qualifications

Additional Required Qualifications

• The candidate must have 2-3 years nursing experience with 1-2 years recent community experience (Home Care Experience preferred). This role requires travel; possession of a valid drivers’ license and access to a motor vehicle in good repair for work related purposes as a requirement of this position. You will be required to operate a laptop computer and mobile phone, and for handling (lifting, lowering, carrying, pushing, and pulling) as a regular part of the job. For example: lifting approximately 30 pounds in and out of a vehicle on a regular basis and carrying this weight up and downstairs throughout the day is common within this job. Intermediate level of computer skills; excellent organizational and time management skills with strong interpersonal skills.

Preferred Qualifications

• Baccalaureate degree in Nursing. Case mgmt exp strongly preferred. InterRAI training, continuing care legislation, and decision making and capacity. Exp with an adult pop aged 18–64, chronic disease management, palliative care, mental health, addictions, trauma informed care, interdisciplinary coordination. Direct clinical care inc nursing interventions, med management, wounds, HPTP, ostomy, mobility, transfers, equipment, family assessment, independent community practice.
