Registered Nurse - Home Care Case Manager

Located in the historical community of Fort Macleod and in close proximity to the Rocky Mountains, Fort Macleod Home Care has an opportunity for a dynamic and enthusiastic individual to join our team. Reporting to the Seniors Health- Integrated Home Care Area Manager, the Registered Nurse (RN) Case Manager is responsible for collaborating and organizing comprehensive health care services for clients in a variety of settings. The RN Case Manager has experience in the role of advocate and can balance the needs of the individual, family, and community within organizational and legislative parameters. The Fort Macleod Home Care Team is knowledgeable with a strong commitment to quality patient care. The RN Case Manager has the ability to develop creative approaches to client care and demonstrates sound critical thinking and ethical decision making. The RN Case Manager demonstrates a patient and family centered approach with all client interactions.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well‑being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values.PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Required Qualifications:

Preferred Qualifications:

Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice in community sector will be considered.Preference given to candidates with Recent Community Care Case Management experience (1-3 Years)Recent clinical experience in psychiatric, geriatric and palliative care nursing preferred.Knowledge of and skills in applying Home Care Nursing Practice Standards.Knowledge of Alberta Aids to Daily Living (AADL) Program is desired.
