Alberta Health Services in Bashaw is seeking an experienced Registered Nurse (RN) Case Manager to lead home care for Bashaw and surrounding areas. You will assess client needs, set goals, plan, deliver and coordinate care with the interdisciplinary team to promote safe, holistic patient outcomes.

New grads welcome with training available. Provisional permit holders are paid at entry RN rates, with full permit compensation aligned to UNA agreements; on-call requirements may apply within 30–45

Qualifications

• Completion of an accredited nursing education program.
• Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
• Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.
• On-call availability within 30–45 minutes when required.

Your Opportunity:

If you are an experienced Registered Nurse who likes small town living, this position might be for you! The Bashaw Home Care program serves a large area of clients including the community of Bashaw and surrounding areas. We work closely with community groups, other health care services in the community and as part of a multidisciplinary team. Registered Nurse - Case Manager is responsible for directing the care of clients under the home care program. The Case Manager must have strong interpersonal communication and interdisciplinary team player skills, excellent critical thinking, creativity, resourcefulness, time management, and organization abilities. The Case Manager responsibilities include assessment of client needs, establishing goals for client care delivery, planning, delivering, and coordination of care needs, resources, and services within professional practice standards and home care program guidelines, communication and coordination of client care with the interdisciplinary team; and evaluation of outcomes. New grads will be considered and training is available for the right candidate. Bashaw is situated less than 100 km from Camrose, just north of the beautiful Battle River valley, between Stettler and Camrose. Our town is a quiet family-oriented with a strong mix of all age groups. Bashaw offers a K-12 school, restaurants, grocery store and a community centre.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Required Qualifications:

Experience in wound care with excellent knowledge of wound care principles and wound care product use is an asset. Experience and training in Connect Care is an asset.
