Job Description : Position Summary

Using the nursing process, assesses the resident physical, spiritual, intellectual, emotional, and social needs formulate a nursing diagnosis, plans, implements and evaluates.

Coordinates management of care for all residents in accordance with current professional nursing practice and Provincial and Regional Standards.

Acts as the First Aide Attendant.

Responsibilities

  • Supports gerontological nursing practice at Buchanan Lodge Salvation Army by participating in the development, implementation and evaluation of Clinical Practices.
  • Serves as a role model and clinical resource for nursing Staff by demonstrating clinical expertise, and providing clinical guidance while sharing expertise, oversight, and clinical knowledge.
  • Coordinates the delivery of individualized resident / client care through assessment, planning, implementing, and evaluating care needs.
  • Provides direct care such as administration of medications, application of clinical interventions and assessments ensuring that physicians, family and or community practitioners are updated in a timely manner.
  • Assignment of duties / tasks to care aides, licensed practical nurses, students, preceptors and ensure completion of same with respect to the care provided as required.
  • Maintains complete accurate records in Point Click Care, as well as the hard copy resident charts.
  • Participates in the orientation of new staff supports student Nurses. Integrates the standards of practice as outlined by BCCNM into the care setting Promotes and interprets all established policies and procedures to staff, residents, and visitors.
  • Participates in Continuous Quality Improvement processes.
  • Participates in committee work as appropriate.
  • Maintains current knowledge of professional practice through literature review, educational program attendance and involvement in professional groups, as outlined in continued competency requirements.
  • Assumes in charge / Team Leader responsibility position as required. Attends care conferences and presents nursing assessments and recommendations to the Multi-disciplinary Team.
  • Participates in the development of the resident care plans in enhanced consultation with the resident, the care team, and the family.
  • Provides input to the Clinical Nurse Lead / Director of Care with regards to the development and implementation of Program policies, nursing care audits and continuous quality improvement activities.
  • Communicates new and revised policies and procedures regarding delivery of care to designated staff.
  • Complete Incident Reports and carries out follow-up actions. Forwards reports to the Director of Care through the Clinical Nurse Lead.
  • Participates in the performance and skill assessment of staff as well as provides input on performance management.
  • Collects, compiles and reports statistical and quality improvement data for research and evaluation process.
  • Promotes effective communication with residents, families, visitors, physicians, staff at all levels and inter departmental collaboration, and Multidisciplinary Team Acts as the Infection Control Lead - by communicating and directing Nursing Team by following the Medical Health Officers Orders as they pertain to any and all Infection control risks / Enhanced Monitoring scenarios / Outbreaks.
  • Assumes the Leadership role in emergency situations ( may include following the Fire and Disaster responsibilities).
  • Replaces staff as per HEU and DCNU Collective Agreements in the absence of the Scheduling Team / completes the log for these calls.
  • Delegates and assigns / delegates staffing levels in conjunction with the CNL and DOC as required.
  • With the priority of Resident Safety Acts as the First Aide Attendant for the facility Identifies and recommends training and skill improvements for staff under his / her supervision.
  • Takes responsibility for the ongoing evaluation of other's work Coordinates the care provided by others and takes responsibility for the active and on going monitoring and follow up of others : accountable for the outcomes of the care provided by others.
  • Informs the On-Call Manager of any emergency including :
  • Staff, resident, or visitor injury if serious
  • Any physical plant concerns regarding a resident or family
  • Urgent concerns regarding a resident or family
  • Missing resident
  • Unexpected death of a resident
  • Fire and disaster situations
  • Completes both internal and Reportable Incidents Acts as a support / mentor to LPN's and HCA's through mentoring and support in the moment at the Point of Care. Perform other related duties as assigned. MDS / AIS completion and delegation.

Qualifications

1. Education and Experience :

  • Graduate of an approved School of Nursing.
  • Current practicing registration with the British Columbia College of Nurses and Midwives BCCNM.
  • Current certificate in First Aide & CPR.
  • Experience in long term care or community-based program preferred.

2. Skills and Abilities :

  • Demonstrated leadership ability.
  • Ability to communicate effectively both verbally and in writing.
  • Physical ability to carry out the duties of the position.
  • Maintains Professionalism in all interactions with residents, family, and staff.
  • Knowledge of community and facility health care systems serving the geriatric resident.
  • Ability to operate related equipment.
  • Aware of licensing requirements and infection control standards
  • Sensitivity and a passion for meeting the needs of the elderly and an ability to interact with residents, family, staff, and members of the community.

Compensation :

The target hiring range for this position is $45.03 to $52.28 with a maximum of $59.52.

Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
