Job summary

Thames Valley Family Health Team in London, Ontario, is seeking a Dietitian to deliver clinical counseling and community nutrition programs within an interdisciplinary primary care setting. The role emphasizes health promotion, risk reduction, and program development with opportunities to lead group education and collaborate with nurses, physicians, and other professionals across multiple sites.

Applicants must be licensed, have 3–5 years of experience, and be able to work across the London

Qualifications

  • Three to five years clinical counseling and community nutrition experience.
  • Experience in program development, implementation, monitoring and evaluation.
  • Member in good standing with College of Dietitians.
  • Experience with health promotion and risk reduction.

Responsibilities

  • Provide clinical nutrition counseling to individuals and groups.
  • Develop, implement and evaluate nutrition programs in the community.
  • Lead nutrition education sessions and workshops.
  • Collaborate with interdisciplinary health teams using EMR.

Job description

The Thames Valley Family Health Team is one of the largest Family Health Teams (FHT) in Ontario. With more than 125 physicians across 21 sites within London/Middlesex, Oxford and Elgin counties, the Thames Valley Family Health Team is committed to providing comprehensive, collaborative primary health care.

Family Health Team staff includes nurse practitioners, registered nurses, social workers, pharmacists, dietitians, occupational therapists and other health care professionals who will work with physicians as an interdisciplinary team, each using their experience and skills to provide patient centred care. This position would initially be located in London, Ontario.

Requirements
  • Academic preparation from an accredited Canadian university program in foods & nutrition or equivalent
  • Practical training in an accredited practical program in Canada or equivalent
  • Three to five years clinical counseling and community nutrition experience or comparable skills, experience and education
  • Experience in program development, implementation, monitoring and evaluation
  • Member in good standing with College of Dietitians
Skills / Abilities
  • Knowledge and proficiency in current, evidenced-based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction
  • Strong leadership, organization, evaluation, time management, communication and interpersonal skills
  • Demonstrates some research experience
  • Experience in the use of EMR and Microsoft Office applications
  • Desire and ability to update knowledge and skills through various means including technology-based opportunities, courses, workshops and conferences
  • Multi-tasking skills related to the delivery of efficient primary care
  • Excellent verbal and written communication skills
  • Exhibit an ability to be open and non-judgmental
  • Knowledge of the goals and structure of the organization
  • Solid understanding and experience with adult group learning principles, teaching self management techniques & ability to evoke participation in a group environment
  • Level of comfort or willingness to become familiar with virtual meeting technology
  • Experience with facilitation of an online program considered an asset
  • Experience and commitment to working collaboratively with other healthcare professionals
  • Experience with disordered eating
  • Experience in working with patients with a history of trauma is an asset, and experience in trauma informed care.

Hourly Rate: $34.8138 - $42.4559 (dependent on experience)
