The role involves holistic, person-centred nutrition planning, collaboration with interdisciplinary teams, and education for patients and caregivers in a community setting.

Qualifications

• Bachelor’s or Master’s Degree in an accredited Food & Nutrition program.
• Registration with the College of Dietitians of Ontario required.
• Member in good standing with the Dietitians of Canada preferred.
• Minimum 3–5 years' experience as a Registered Dietitian.
• Experience working in a community setting.
• Ability to work autonomously and as part of an integrated health care team.
• Valid Ontario driver's license and a reliable vehicle.
• Compensation is based on experience; Pay per visit model.

Responsibilities

• Assesses dietary needs unique to each person and family, considering social determinants of health and cultural factors.
• Assesses the person’s and caregivers’ skills and need for dietary education and supports.
• Creates a holistic, person-centred plan acknowledging psychosocial impacts of dietary changes.
• Provides education, advice and practical strategies to manage dietary symptoms and nutrition support.
• Engages in dysphagia assessment and management.
• Collaborates with interdisciplinary teams including physicians, nurses, therapists and social workers.
• Contributes to team meetings and care planning conferences.
