Extendicare is seeking a Registered Dietitian to lead nutrition care across our Canadian long-term care homes. You will assess residents, develop and monitor nutrition plans, and educate staff and families to support optimum nutrition outcomes.

You will collaborate with an interdisciplinary care team, review menus, oversee nutrition programs, and ensure compliance with regulatory standards. Competitive compensation and comprehensive benefits are offered.

Qualifications

• Must have degree in Food and Nutrition from an approved university.
• Current registration in the applicable provincial College of Dietitians.
• Membership in Dietitians of Canada and the Gerontology Network.
• Knowledge of Provincial Food Premises Regulation, CCHSA Accreditation Canada, and Residents' Bill of Rights.
• Strong written and oral communication, critical thinking, care coordination, time management and customer focus.
• Proficient computer skills with MS Office, Synergy, and POC/eMAR.
• Vulnerable Sector Check current within six months.

Responsabilités

• Assess nutrition status for residents on admission, annually, and after status changes.
• Determine nutrition and hydration risk to prescribe/intervene as needed.
• Develop nutrition plan of care for newly admitted residents.
• Evaluate and modify nutrition plans with RD as needed.
• Document nutrition care for residents followed by RD.
• Provide nutrition/diet counseling for residents and caregivers.
• Monitor high/moderate risk residents per RD monitoring and guidance.
• Participate in interdisciplinary resident care team and conferences.
• Join committees such as Pharmacy & Therapeutics, Resident Care/Professional Advisory and Ethics.
• Contribute to Care Teams like Skin/Wound, Bowel Management and Dysphagia.
• Coordinate quality improvement of nutrition/hydration and dining with NM.
• Provide in-service education for staff.
• Review and approve seasonal menus and therapeutic/texture-modified diets.
• Approve home menus and oversee Restorative Feeding and Enteral Nutrition programs.
• Collaborate with NM and advise Administration on resident nutrition needs including language, culture, and preferences.
• Uphold Extendicare values and Code of Conduct; complete other duties as assigned.
