Overview

This position reports to the Professional Practice Manager and is primarily responsible for supporting outpatient Diabetes and Metabolic Health clinics within Michael Garron Hospital. Additionally, it supports the offsite transitional care unit at Kew Beach Retirement Home at 500 Kingston Road once a week. The role may include other responsibilities as needed.

The Registered Dietitian is a vital part of the interdisciplinary team, providing collaborative, comprehensive, and person-centered care, education, and behavior change strategies to improve quality of life across all ages. Responsibilities include delivering diabetes education, nutritional therapy, and prevention strategies for managing T1 and T2 diabetes in adults, gestational diabetes, and cardiometabolic and obesity care. The Dietitian will also conduct ongoing assessments, develop nutritional care plans, and promote self-management to achieve personalized behavioral and treatment goals, reducing risks and optimizing health outcomes.

The role requires expertise in diabetes education, medical nutrition therapy, and adult nutrition care. The Dietitian will collaborate with the team to advise clients on diet and insulin management, identify educational needs, develop educational programs, and support other dietitians and team members in complex cases. The position involves occasional cross-coverage in inpatient acute care, complex continuing care, and metabolic/bariatric clinics, including coverage at Kew Beach.

Education

• Bachelor's Degree in Nutrition or equivalent.
• Registration with the College of Dietitians of Ontario in good standing.
• Registration with Dietitians of Canada preferred.
• Certified diabetes educator certification preferred.

Qualifications

• Minimum 2 years recent clinical experience in hospital settings, with expertise in complex diabetes management preferred.
• Experience with metabolic and bariatric conditions is an asset.
• Certified Insulin Pump Trainer is preferred.
• Strong collaboration, communication, and educational skills.
• Ability to work effectively in a fast-paced environment as part of a team.
• Proficiency in evidence-based practice and professional development.
• Computer skills (Microsoft Office).
