Southbridge, owner and operator of several long-term care homes and retirement communities in Southwestern Ontario, is seeking a Registered Dietitian to join our dynamic Food and Nutrition Department team. Our team is dedicated to providing residents with nutritious, delicious meals prepared and served with love. This casual position covers vacation and leave. The successful candidate will be expected to travel to Southbridge LTC homes in Southwestern Ontario. The role offers flexibility with in-person, hybrid, and remote work arrangements, determined by business needs.

Responsibilities:
  • Consult and advise the Food Service Manager and Director of Care on nutrition and hydration program needs as a member of the Clinical Team.
  • Conduct nutritional assessments for residents using RAI MDS 2.0 as required.
  • Develop, implement, and evaluate individualized nutritional care plans, involving residents, families, and the care team.
  • Maintain excellent communication with the Food Service Manager regarding residents' changing needs.
  • Review seasonal menus for nutritional adequacy and provide written approval.
  • Educate residents, families, and team members on nutrition and hydration.
  • Participate in interdisciplinary teams, regional, and professional activities as appropriate.
  • Contribute to committees related to resident care, facility operation, and quality indicators.
Qualifications:
  • Registered Dietitian eligible for College of Dietitians of Ontario membership.
  • Extensive knowledge of geriatric nutrition.
  • 3-5 years of experience in a long-term care setting preferred.
  • Proficiency with Microsoft Office, internet, and familiarity with RAI MDS 2.0; experience with PointClickCare is an asset.
  • Excellent organizational, time management, and multitasking skills.
  • Knowledge of Occupational Health & Safety practices and legislation.
  • Strong oral and written communication skills.
  • Customer-focused attitude with the ability to build effective team relationships with staff and residents.
