Job summary

Chosen Care Group Ltd. is seeking experienced Care Workers to provide specialist complex care at home for individuals with advanced neurological conditions, including tracheostomy and ventilator management, PEG feeding, and comprehensive personal care.

You will deliver person-centred care, support daily living activities, and work within a dedicated complex care team with ongoing supervision and training.

Qualifications

• Enhanced DBS check (Adults and Children)
• Minimum two professional references covering the last 3 years
• Verification of identity and right to work in the UK
• Verification of relevant health and social care qualifications
• Declaration of any cautions, convictions, or professional sanctions
• Confirmation of eligibility to work with vulnerable adults

Responsibilities

• Deliver person-centred care in line with care plans
• Full support with all activities of daily living
• Tracheostomy care and management
• Use and monitoring of cough assist machines
• NIPPY 4+ ventilator support
• Humidification processes
• Oral and deep suctioning techniques
• Oxygen therapy administration
• PEG feeding and medication administration via PEG
• Monitor condition and respond to changes appropriately
• Collaborate with family and healthcare professionals
• Adhere to strict infection control procedures
