Job Description

The Central West Ontario Health Team (CW OHT) is a collaboration of health partners across Brampton, North Etobicoke, West Woodbridge, Malton and Bramalea. As one of the largest OHTs in the province, we serve close to 900,000 people. Over 153 partners including home and community care, acute care, specialty care, primary care, long-term care, mental health and addictions services, patients/clients, and community support services are working together to provide better coordinated care for the community.

Reporting to the Manager, Integrated Health Systems, the CW OHT Project Manager will collaborate with OHT partners, OHT Secretariat team members, and the collaboration council to lead and manage priority programs and initiatives. These initiatives are guided by the 2024-27 CW OHT strategic plan and the annual business plan, supporting the development of the OHT towards maturity and an integrated, population health management approach to care. The Project Manager will be responsible for leading planning, development, and execution of key initiatives, as well as overseeing working groups and committees.

Accountabilities:

  1. Consult with and provide leadership support to key stakeholders in strategy development and execution of OHT projects/initiatives in alignment with the CW OHT strategic plan
  2. Lead project plan development and implementation with clear communication on milestones, deliverables, and accountabilities
  3. Develop budgets and operating plans
  4. Create structures, processes, and tools to enable integrated delivery of results
  5. Support monitoring and evaluation of projects, ensuring quality improvement
  6. Foster cross-functional work streams with stakeholders to ensure alignment
  7. Apply change management principles to educate teams through project complexities
  8. Build strong relationships with the collaboration council, partners, Ontario Health, and other stakeholders
  9. Monitor, track, escalate, and manage risks with strategies for mitigation
  10. Use the Quintuple Aim framework to monitor patient, project, and system impacts
  11. Perform other duties as assigned

Qualifications:

  1. University degree in a related field (public health, health promotion, health sciences, sociology, health/business administration) or an acceptable equivalent
  2. Master’s degree in a related field
  3. At least five years of project management experience
  4. Healthcare experience preferred
  5. Project Management certification is an asset
  6. Lean Six Sigma and Change Management certifications are assets
  7. Experience in developing strategic initiatives, budgets, and reports
  8. Data collection, analysis, and health equity frameworks experience
  9. Knowledge of continuous quality improvement methodologies
  10. Experience leading clinical projects is an asset
  11. Partnership development and stakeholder engagement skills
  12. Experience with project planning and implementation in healthcare
  13. Expertise in project management, quality improvement, change, and risk management processes
  14. Strong relationship-building skills
  15. Innovative, motivated, with coaching and mentoring abilities
  16. Conflict resolution skills and entrepreneurial problem-solving mindset
  17. Commitment to equity, diversity, inclusion, and anti-racism
  18. Understanding of Ontario healthcare, including Ontario Health Teams
  19. Proficiency in Microsoft Office, Project, and Visio

Additional Information:

Hours: Days, 8-hour shifts, including weekends and holidays (subject to change)

Annual Salary: $85,839 - $107,308.50
