Job summary

Yee Hong Centre for Geriatric Care is seeking a dedicated facilitator to deliver holistic, relationship-centered programs for clients in our Community Care and Wellness Hub in Toronto. Qualifications

  • Diploma in a health or social services discipline or equivalent.
  • Minimum three years of experience in social services or community health.
  • Experience in a geriatric care setting and working with older adults is essential.
  • Completion of a current CPR certification.
  • Strong interpersonal and relationship-building skills with multidisciplinary teams.
  • Satisfactory Police Reference Check with vulnerable sector screen.
  • Good oral and written communication in English and Chinese.
  • Proficient computer skills and experience with digital documentation.

Job description

Under the direction of the Program Coordinator, facilitates activities and ensures the delivery of high-quality services to clients of the Community Care and Wellness Hub, in accordance with the organization's relationship-centered care approach, relevant government standards, client’s rights and the organization’s mission, vision and philosophy.

Qualifications:

  • Diploma in a relevant health (e.g., therapeutic recreation, kinesiology, nursing) or social services (e.g., social service work, community services, gerontology) discipline or equivalent.
  • Minimum three years of experience in social services or community health (e.g., activation, rehabilitation, health education).
  • Experience in a geriatric care setting and working with older adults is essential.
  • Completion of a current C.P.R. certification.
  • Strong interpersonal and relationship‑building skills, with an ability to collaborate in a multidisciplinary team.
  • Satisfactory Police Reference Check (with vulnerable sector screen) result.
  • Good oral and written communication skills in English and Chinese.

Responsibilities:

  • Under the overall direction of the Program Coordinator, participates in the planning and delivery of holistic, relationship-centered program services.
  • Develops and implements the monthly program schedule within established guidelines and clinical frameworks.
  • Plans and facilitates daily program activities and social-recreational sessions to ensure high engagement and adherence to the monthly schedule.
  • Facilitates and ensures the smooth operation of daily activities in a safe, hygienic, and supportive environment.
  • Provides timely and appropriate support and behavior management to clients during the programing, contributing to the fulfillment of client’s physical, social, and emotional needs.
  • Responds to client’s direct care needs including medical emergencies or behavioral incidents, ensuring appropriate action is taken and documented in accordance with organization's protocols.
  • Collaborates with the team to distribute, portion, and serve meals, while ensuring meal assistance is provided in accordance with the dietary needs, textures, and clinical requirements outlined in the client’s care plan.
  • Provides timely support, information, follow‑up and outreach to clients and caregivers to ensure social, recreational, and developmental needs are met.
  • Provides day‑to‑day guidance, orientation, and support to volunteers within the program setting.
  • Collaborates with the team to synthesize information during huddles and debriefings, sharing frontline observations and supporting the implementation of individualized care approaches, including daily goals and dietary needs, within program activities.
  • Liaises and collaborates with multidisciplinary healthcare professionals and supports the Program Coordinator to ensure program delivery is adapted to meet the evolving needs of the clients.
  • Complies with all risk management policies, including health and safety standards and infection control protocols.
  • Maintains up-to-date and accurate files, records, and statistics of the services and clients.
  • Collects program fees and maintains accurate records of inventory, fees, and expenditures.
  • Maintains professional knowledge by keeping up-to-date with developments and best practices in the sector.
  • Other duties as assigned.
