Job summary

Yee Hong Centre for Geriatric Care in Greater Toronto Area is seeking an Activation Worker (Casual part-time) to plan and conduct daily activation, recreational and social programs for residents in a quality-driven environment.

The role requires a Diploma in recreation or related field, experience in geriatric settings, and strong customer service with Chinese language skills preferred. CPR and Food Handler certificates, and a vulnerable sector check are required.

Qualifications

  • Diploma or equivalent in recreation/therapeutic recreation/kineseiology or related field.
  • Experience in social services or long-term care is an asset.
  • Experience in facilitating activation and therapeutic recreation programs for geriatric clients.
  • Creativity and innovation within budget constraints.
  • Excellent customer service and positive team contribution.
  • Knowledge of Chinese language is an asset; CPR and safety certifications required.

Responsibilities

  • Plan and conduct daily activation, recreational and social programs for residents.
  • Assess residents on admission and quarterly; monitor care plans.
  • Supervise volunteers and mentor students and interdisciplinary team members.
  • Maintain resident records and contribute to inter-disciplinary care.
  • Ensure activities are safe, hygienic and resident-centered; manage costs and supplies.

Job description

Activation Worker (Casual part-time) Under the supervision of the Activation Manager, the Activation Worker plans and conducts daily activation, recreational and social programs for residents of the LTC Home that reflects the organization's resident centred approach to improve and maintain the residents' quality of life in the Home.

Responsibilities
  • Conducts timely and accurate assessment of residents upon admission and on a quarterly basis.
  • Prepares and monitors effectiveness of care plan
  • Plans and implements innovative regular and seasonal recreational and social activities to improve residents' quality of life, taking into consideration their feedback, needs and preference, and ensures activities are conducted in a safe, hygienic and supportive environment, including one-on-ones, small group and large group activities
  • Assists the Activation Manager in supervising volunteers who assist or facilitate activation programming
  • Mentors and works effectively with students and interdisciplinary team members
  • Maintains and updates resident records
  • Works with staff of other disciplines in the LTC Home in providing quality care to LTC Home residents
  • Shops for cost effective and appropriate program supplies and maintains accurate record of fees and expenditure
  • Assists the Activation Manager in providing information and education to residents and their family members (hand hygiene workshop)
  • Participates in in-service programs and professional development
  • Keeps up-to-date with developments and best practices in the field
  • Other duties as assigned
Qualifications
  • Diploma in Recreation and leisure studies, therapeutic recreation, kinesiology, or equivalent
  • Experience in a social services or long-term care setting an asset
  • Experience and skills in facilitating activation and therapeutic recreation programs, preferably working with the geriatric population
  • Creativity and innovativeness in developing meaningful programs for clients within a tight budget
  • Superb customer service skills.
  • Demonstrated initiative to support the clients and team in a positive and efficient manner.
  • Computer literacy.
  • Knowledge of Chinese word processing, an asset
  • Current CPR Certificate
  • Current Food Handler Safety Certificate
  • Satisfactory Police Vulnerable Sector Screen
  • Ability to communicate in English and Chinese
Physical Demands/Working Conditions

  • Occasional travel within the Greater Toronto Area
  • Use of own Vehicle

Salary Range: $21.97 - $24.51
