Yee Hong Centre for Geriatric Care in Markham, Ontario is seeking a part-time Program Support role for the Day Program. The incumbent will plan activities, ensure a safe, hygienic and supportive environment and respond to crises to meet clients’ physical, social and emotional needs.
They will coordinate referrals and outreach, work with clients and caregivers on care plan design, supervise volunteers, and maintain accurate records and fees. CPR and vulnerable sector checks are required.
Under the direction of the Program Manager/Program Coordinator, plans or participates in the planning of appropriate activities and providing high quality services to clients of the Day Program.
Diploma in Social Work or Social Services, health sciences, activation, or equivalent
Experience in geriatric rehabilitation is essential
Excellent customer service skills
Ability to work independently and contribute effectively as a team player
Computer literacy in Microsoft Office Suite
Recent certification in C.P.R.
Recent satisfactory Police Vulnerable Sector Screen result
Fluency in English and ability to converse in Chinese
Must be able to work across different Yee Hong sites as needed
Some lifting and transferring required
This is a temporary part time position for 1 year, with possibility of extension
Salary Range: $21.86 - $25.72
