Job summary

South East Grey Community Health Centre (SEGCHC) is seeking a motivated Registered Practical Nurse to join an interdisciplinary primary care team. This approximately 20-month contract offers 0.8–1.0 FTE (30–37.5 hours/week) with primary placement in Markdale and occasional coverage at Chatsworth, Ontario.

Qualifications

• Diploma in Practical Nursing from a recognized institution.

Responsibilities

• Conduct patient assessments, triage, health education and counselling.
• Support safe, patient-centered care within a collaborative primary care environment.
• Maintain clinical rooms, equipment, supplies, and resources to ensure a safe, organized, and efficient care environment.
• Completing accurate and timely clinical documentation, participating in chart reviews and case conferences, and contributing to effective care planning.
• Advocating for patients by connecting them with appropriate internal services, community resources, and partner organizations.
• Contributing to the development, implementation, monitoring and evaluation of clinical programs and services that support individuals, families, and communities.
• Collaborating with physicians, nurse practitioners, nurses, administrative staff, and other members of the interdisciplinary team to promote positive patient outcomes.
• Demonstrating flexibility and the ability to provide coverage at multiple clinic locations as required.
