We are seeking a dedicated and compassionate Primary Care Nurse Practitioner to join our growing family practice. You will provide comprehensive care to patients of all ages—focusing on prevention, chronic disease management, and patient education.

Duties

  • Manage chronic illnesses (e.g., diabetes, hypertension) in a primary care setting.
  • Conduct physical exams, well baby visits, PAP tests, immunizations, and preventative care.
  • Order and interpret diagnostic tests within NP scope.
  • Counsel patients and families on disease prevention, health promotion, and lifestyle modification.
  • Complete medical, government, and workplace forms.
  • Maintain accurate EMR documentation and manage incoming results and reports.
  • Coordinate referrals to specialists and support patients in navigating the health system.
  • Collaborate with other healthcare professionals to ensure coordinated patient care.

Requirements

  • Master's degree in Nursing.
  • Valid Nurse Practitioner license with certification in primary care.
  • Current registration with the College of Nurses of Ontario (RN-EC).
  • Strong background in acute care settings is an asset.
  • Familiarity with electronic medical records systems, particularly Oscar Pro.
  • Excellent interpersonal skills with a focus on patient-centered care.
  • Ability to work effectively in a fast-paced environment while maintaining attention to detail.
  • Commitment to ongoing professional development and staying current with best practices in primary care.
  • Potential : May increase to 4 days per week by the end of the year
  • Remuneration : Can be negotiated based on experience
