We are presently seeking an enthusiastic individual to join our high-functioning interprofessional team as a Nurse Practitioner with a specialized focus on our geriatric patient population. We are taking an exciting and innovative approach with our virtual care platform and are looking for someone with the drive and energy to help promote further innovation and improve client care. Evening and overnight call coverage will be required.

Responsibilities

• Perform advanced and comprehensive health assessments
• Use evidence-based practice to formulate diagnoses and establish comprehensive, holistic, therapeutic care plans.
• Utilize virtual care software effectively and adapt to fast-paced technological improvements
• Provide clinical care through a virtual hub and obtain consent for treatment
• Diagnose illnesses and perform interventions within the NP scope of practice
• Order and interpret diagnostic tests and prescribe medications
• Manage disease-specific pain and symptoms
• Follow up with clients, especially when the primary appointment was initiated by another NP or MD
• Identify, assess, and meet educational needs of clients, families, and caregivers
• Provide client education, coaching, and counselling for acute and chronic conditions
• Participate in program development, monitoring, and evaluation through regular meetings
• Be available for on-call and after-hours services for Rhynocare clients
• Provide leadership in critical thinking, ethical decision-making, and evidence-informed service planning

Qualifications

• Completion of a recognized Nurse Practitioner program
• Primary Health Care Nurse Practitioner preferred
• Current registration with the College of Nurses of Ontario in the Extended Class
• Minimum 3 years nursing experience, including 2 years as an NP
• BLS-HCP certification
• Ability to speak a second language is an asset
• Access to a home office
