Bayshore Medical Personnel is seeking a Primary Care Nurse (PCN) to deliver primary healthcare services in Borden, Ontario. PCNs are responsible for patient assessment, screening, health education, and managing chronic diseases to improve health outcomes and facilitate access to health services.

As part of a multidisciplinary healthcare team, PCNs develop and implement care plans and provide follow-up for patients with acute or chronic physical, mental, and occupational health needs.

Qualifications

Successful candidates must have at least one (1) year of full-time experience in family practice, ambulatory care, or a similar healthcare setting.

Must Possess

• Baccalaureate Degree in Nursing from a recognized university.
• Registered Nurse license from the relevant provincial or territorial professional body.
• Certificate of Basic Life Support (BLS) for Healthcare Providers or CPR/AED Level C.

Additional Requirements

• Current N95 mask fit test certificate (CSA Standards Z94.4) valid for two years.
• Workplace Hazardous Materials Information System (WHMIS) certification valid for two years.
