Job summary

Lifepoint Health is seeking a Physical Therapist (PT) for a Per Diem position at Kindred Hospital Ontario, CA. The role focuses on evaluating patients, developing treatment plans, and advancing recovery while coordinating with nursing and family members.

Responsibilities include documenting care, communicating progress to the team, and ensuring safe operation of equipment. CPR certification and a current PT license are required; rehab experience is preferred.

Qualifications

  • Graduate of an accredited physical therapy program.
  • Current PT license.
  • CPR certification required.
  • Experience in rehab setting preferred.

Responsibilities

  • Evaluate a patient’s condition and develop a treatment plan, guiding progress day by day.
  • Instruct nursing staff and patients’ families on follow-through programs.
  • Communicate patient progress to supervisor and team; assist with scheduling and post charges.
  • Document patient care per regulatory and accrediting requirements.
  • Instruct family or nursing staff in follow-through programs.
  • Maintain equipment and work area in a safe, clean condition.
  • Adhere to company standards and applicable laws.

How You'll Contribute
  • Put your physical therapy skills to work where they're really needed -evaluate a patient's condition, develop a treatment plan, and help them get better, day by day. You'll also instruct the nursing staff and the patient's families on follow-through programs that build on the progress they've made
  • Communicate patient progress or problems to supervisor and other team members; assist with patient scheduling and post charges daily to patient records
  • Document patient care in accordance with regulatory, licensing, payer and accrediting requirements
  • Instruct patient's family or nursing staff in follow-through programs
  • Maintain equipment and work area in a safe and clean condition
  • Handle job responsibilities in accordance with the Company's Code of Business Conduct, the Corporate Compliance Agreement, appropriate professional standard and applicable state/federal laws
  • Other duties as assigned
