Job Description
Reporting to the Pharmacy Manager and under the direction of the Pharmacy Professional Practice Leader, the Pharmacist will provide consultative and professional pharmaceutical services. They will assist in coordinating the activities of the Pharmacy department, oversee drug distribution activities, and ensure drugs are procured, stored, prepared, prescribed, dispensed, and administered safely and effectively. The Pharmacist will also provide clinical pharmacy services, including comprehensive drug information to support patient care, and promote appropriate pharmacotherapy to optimize patient outcomes. Additionally, they will participate in educational activities related to their practice area and collaborate with clinical, nursing, and allied health staff to deliver pharmaceutical care across the continuum of care.
Skills and Qualifications
- Bachelor of Science in Pharmacy or PharmD
- Current, unrestricted registration with the Ontario College of Pharmacists
- Successful completion of a Canadian Hospital Pharmacy Residency Program (preferred)
- Minimum of three years hospital pharmacy practice experience (preferred)
Responsibilities
- Perform therapeutic reviews of medication orders and prescriptions for accuracy and appropriateness
- Identify and communicate drug-related problems to pharmacists or physicians
- Perform final checks on new medication orders and prescriptions
- Supervise pharmacy assistants and technicians in drug distribution activities
- Provide direct client care as a clinical pharmacist
- Identify, resolve, and prevent drug-related problems collaboratively
- Individualize medication regimens considering pharmacodynamics and pharmacokinetics, interpret serum drug levels, and make recommendations
- Conduct medication histories, assess adherence, and suggest therapy modifications
- Participate in interprofessional rounds and conferences to evaluate patient progress and discharge planning
