JOB SUMMARY
Under the supervision of the Pharmacy Operations Manager and/or Pharmacy Operations Supervisor, performs all duties necessary to ensure the efficient daily operation of the pharmacy, providing quality products and clinical services to home and community care patients.
DUTIES AND RESPONSIBILITIES
- Performs and documents all activities of dispensing medication, including clinical review, pre-check, visual verification of sterile compounding, and final check of prepared product according to Bayshore Policies and Procedures (P&P) and standards outlined by the National Association of Pharmacy Regulatory Authorities (NAPRA) and adopted by the Ontario College of Pharmacists (OCP):
- Reviews medical orders for clinical appropriateness, considering bloodwork, medication list, diagnosis, and guidelines.
- Prepares and verifies infusion supply orders for accuracy.
- Prepares and verifies compounding instructions and infusion pump programming.
- Oversees sterile compounding with visual verification.
- Checks final sterile compounded products for particulates and labeling standards.
- Obtains, interprets, and evaluates medical orders for therapeutic appropriateness, accuracy, and completeness.
- Confirms the accuracy and final check of compounded medications and pump programming.
- Ensures proper supplies are ordered and data entry is accurate before dispensing.
- Ensures compliance with all relevant laws, regulations, and standards.
- Adheres to guidelines for Beyond Use Date and quality control procedures.
- Performs accurate prescription entry, labeling, and dispensing as per regulations.
- Collaborates with healthcare providers for pharmaceutical and clinical input.
- Maintains documentation per policies and legislation, including all legally required records.
- Provides clinical and technical information to patients and care teams regarding medications and equipment.
- Documents patient-related communications in the software.
- Notifies healthcare team of therapy changes.
- Follows procedures for obtaining medications, equipment, and supplies, maintaining inventory levels.
- Provides patient counseling on new prescriptions and updates medication lists.
- Reviews prescriptions for ongoing treatment needs.
- Works with and delegates to team members including pharmacy staff, healthcare providers, and support staff.
- Monitors performance metrics and participates in quality improvement activities.
- Attends meetings and participates in training and orientation of new staff.
- Assists with narcotics tracking and medication reconciliation as required.
- Performs other duties as assigned.

Education and/or Experience
- BSc in Pharmacy or PharmD from an accredited institution.
- Experience in sterile compounding is an asset.
Language Skills
- Ability to read, interpret professional and regulatory documents.
- Effective written and verbal communication with healthcare teams and patients.
Mathematical Skills
- Accurate pharmaceutical calculations, including conversions and percentage calculations.
Reasoning Ability
- Problem-solving skills and ability to interpret instructions.
Certificates, Licenses, Registrations
- Licensed with Ontario College of Pharmacists in good standing.
- Current CPR certification.
- Sterile compounding certificate is an asset.
Other Skills and Abilities
- Proficiency with Microsoft Office; experience with Kroll is an asset.
