Accountability Objective

Under the direction of the Health and Wellness Manager, participates in the delivery of care to residents. Delivers care based on individualized care plans, policies and procedures, and established standards of care.

Background / Qualifications

Has obtained a certificate from an approved Health Care Aide or Personal Support Worker program. Has completed a minimum of grade 12. Preference given to individuals with experience in long-term care or retirement residences. Fluency in written and verbal English is required. A resident-oriented individual whose values and vision align with those of Diversicare.

Key Responsibilities

1. Participates in the Verve Quality Improvement (VQI) program.
2. Provides care to residents, including cognitive, social, and emotional support, according to their care plan and in a manner that promotes comfort and safety.
3. Performs nursing care procedures following standards of resident care, routine practices, and infection control protocols (e.g., outbreak management).
4. Complies with relevant legislation such as the Occupational Health and Safety Act, Workplace Safety and Insurance Act, and Workplace Hazardous Materials Information System. Recognizes hazards, reports incidents, and participates in in-services and fire drills. Adheres to corporate policies and procedures.
5. Completes all mandatory training and education requirements.
6. Accurately and legibly documents nursing care and maintains health records in compliance with privacy legislation.
7. Monitors and evaluates intervention outcomes, suggesting improvements as needed.
8. Performs other tasks as delegated by the Unregulated Care Provider (UCP), Registered Practical Nurse (RPN), Health and Wellness Manager, or General Manager.
