Job summary

Southbridge Care Homes is seeking a Personal Support Worker at Westside LTC in Etobicoke, ON. The role involves direct resident care with mixed shifts and on-call casual status.

You will support residents with daily activities, meals, and emotional needs, reporting to the Director of Care. A PSW program meeting Ontario standards (600 hours) is required and previous long-term care experience is an asset.

Qualifications

• Completed a PSW program meeting Ontario standards; minimum 600 hours.
• Experience in long-term care is an asset.
• Knowledge and experience with care planning systems and documentation.
• Able to read and write English and understand English instructions.
• Physically and mentally able to perform duties and work with residents and families.

Responsibilities

• Assist residents with ambulation, feeding, bathing and grooming.
• Encourage participation in activities and assist with transportation to events.
• Assist with meal service and nourishment delivery to residents and provide feeding assistance as required.
• Provide emotional and social support to residents and families including palliative care.
• Follow established policies on documentation, quality assurance, fire safety, infection control.
• Communicate unit activities and resident care needs to supervisor.
• Participate in care conferences and documentation related to resident care plans.

Position: Personal Support Worker

Status: Casual (On call)

Reports to: Director of Care

The Personal Support Worker provides direct personal and nursing care to residents to fulfil the physical, intellectual, emotional, social and spiritual needs of residents in accordance to home polices, procedure and work routines.

Key Responsibilities:

• Assists in providing direct care for residents; assists with ambulation, feeding, bathing and grooming.
• Encourages participation in activity programs by assisting residents to and from activities.
• Assists with meal service and nourishment delivery to residents and provides feeding assistance as required.
• Provides emotional and social support to residents/families including palliative care.
• Adheres to established departmental polices and procedures regarding documentation, quality assurance, fire, safety, environment and infection control.
• Consults with and informs supervisor of unit activities, needs and matters related to resident care.
• Participates in care conferences and documentation related to resident care plans.

Knowledge and Skills Required:

• Completed a PSW Program that meets the appropriate standards established by the Ministry of Training, Colleges and Universities, the National Association of Career Colleges, or the Ontario Community Support Association and must be a minimum of 600 hours in duration (including class time and practical experience time)
• Experience in long-term care an asset
• Knowledge and experience in care planning systems and documentation required
• Mentally and physically able to perform duties as assigned
• Ability to communicate positively with other staff, residents and families
• Demonstrates empathy, concern and interest in providing quality care and service to residents
• Demonstrates organizational skills and ability to complete assignments in a timely fashion
• Ability to work independently and as part of a team
• Ability to provide resident care in accordance with application legislation, job routines, nursing care plans, ability to read and write English and understand verbal and written instructions in English.

Additional Requirements include

• A negative vulnerable sector check completed within 6 months of start date.

• Completion of a 2 Step TB Skin test within the last 12 months of start date.
