Responsibilities

• Communicate with the interprofessional care team regarding patient discharge and community transition needs.
• Provide crisis counselling and short-term follow-up to reduce hospital demand.
• Refer to community mental health and related services per established protocols.
• Maintain client records in clinical information systems (e.g., PARIS).
• Participate in quality improvement activities and professional development.
