Position summaryDepartment / Research Program: Child Health and Human Development Program
We are seeking an individual that will support first and second-generation migrant families with children and/or youth with special healthcare needs navigate transitions within and across systems. Special healthcare needs include chronic conditions like asthma, diabetes, sickle cell, autism, and others.
The incumbent will be working with families and their children with special healthcare needs to liaise with their care providers and care systems to help improve access to care by:
- Facilitating communication between health and service providers and families
- Supporting and empowering families and youth with navigating the healthcare and social services systems
- Providing direct assistance with completing application and funding forms (Québec family allowance, school registration forms)
General DutiesBelow is the approximate division of time and duties.
- 50% Be a primary contact for patients/families with respect to connecting to resources, services, and funding, advocating for care coordination, and liaising with clinical and community providers
- 35% Identify needs and refer to appropriate resources, liaise with community partners as needed
- 10% Support the administrative and data management needs of the program
- 5% Perform any other related duties as requested by his/her immediate supervisor
1. SUPPORTING CARE COORDINATION AND CONNECTING TO RESOURCES● Connect families to health and/or social service resources in the community that are adapted to the relevant needs of refugee and immigrant families (e.g., completing disability funding forms, culturally relevant autism services)
● Facilitate connections between services in an efficient manner while considering the needs and other responsibilities of the families
● Facilitate information sharing between the patient/family and health care professionals in the hospital and community (e.g., acting as liaison, care plans)
● Maintain a list of programs and services relevant to immigrant and refugee children and youth (e.g., identify IFHP-registered services, multilingual services).
2. EMPOWERING AND SUPPORTING FAMILIES● Engage and gather pertinent information to understand patient and family needs
● Provide direct, culturally-safe support to patients and families to facilitate access to services, such as identifying resources, completion of forms/applications, or advocating with a focus on familial empowerment
● Provide support to families to understand the health care and social service systems in Quebec/Ontario.
3. EFFECTIVELY COMMUNICATE● Employ active listening and remain solutions-oriented in interactions with patients, families, and members of the healthcare team
● Communicate with families in a culturally safe manner.
4. ADMINISTRATIVE TASKS● Maintain accurate and complete documentation of all interactions, for example, by using the electronic medical records system
● Monitor the types of services provided for patients and families and encounters with the families
● Collect data as needed, e.g., workload statistics, volumes, referrals, types of needs identified, wait times, etc.
● Maintain project documents, as required
● Perform any other related duties as requested by his/her/their immediate supervisor.
