Position Summary
The Patient Flow Coordinator will work collaboratively between Providence Healthcare service teams and its acute care and community-based referring partners to promote optimal patient flow and to expedite, maximize and monitor service utilization by:
- Identifying and initiating the admission process for patients who are willing to participate in and will benefit from Providence Healthcare services through the continuum of inpatient to outpatient programs;
- Facilitating the repatriation of discharged patients sent from Providence Healthcare (inpatient or outpatient) to the acute care organization as appropriate;
- Promoting communication between Providence Healthcare and referring organizations to support and enhance patient flow by identifying, reducing or removing barriers to access the breadth of services at Providence Healthcare;
- Participating in the monitoring and achievement of relevant operational and strategic targets associated with patient flow and outcomes.
Qualifications/Skills Required
- Extensive relevant clinical experience (>5 years);
- Excellent knowledge of rehabilitation practices care especially as they relate to frail elder, complex and marginalized populations;
- Demonstrated project management skills with proven experience in effective team leadership;
- This position requires the knowledge, skill, and judgment of a Registered Occupational Therapist or Registered Physiotherapist in good standing with their respective professional college in Ontario;
- Knowledge and familiarity with Resource Management and Referral (RM and R), e-stroke application processes, community resources, relevant healthcare legislation and utilization pressures;
- Familiarity with Local Health Integration Networks is an asset;
- Working knowledge/familiarity with widely utilized clinical assessment tools (e.g., NRS, FIM, MoCa, MDS);
- Excellent critical thinking, interpersonal, diplomatic and communication skills (verbal and written) that demonstrate Providence Healthcare values to a variety of organizations with differing needs;
- Excellent problem recognition skills and ability to proactively achieve resolutions both through direction and negotiation and make decisions rapidly;
- Proficiency in computer database, spreadsheet and software applications;
- Willingness to learn about Providence Healthcare and Acute Care programs and cultures;
- Commitment to continuing education and self-directed learning;
- Very good attendance record;
- Good performance record.
