Job summary

Ontario Health atHome in Kingston seeks a Care Coordinator to assess patient needs, determine eligibility, and link clients with services. You will coordinate care plans and work with patients, caregivers and a multidisciplinary team.

The role requires registration with Ontario regulatory bodies, a relevant university degree, 2+ years in community health, and strong communication and organizational skills. A valid driver’s license and access to a vehicle are required, with COVID‑19 vaccination

Qualifications

  • Registration in good standing with the appropriate Ontario regulatory body.
  • University degree in Nursing, Physiotherapy, Occupational Therapy, Speech‑Language Pathology or Social Work.
  • 2+ years of recent experience in community health or a related field.
  • Knowledge of the health care delivery system and community resources.
  • Excellent interpersonal, communication, assessment, problem‑solving, and decision‑making skills.
  • Effective time management and organizational skills; able to work independently in a busy multidisciplinary environment.
  • A valid driver’s licence and access to a reliable vehicle.
  • Proficient in a Windows environment.
  • We have a mandatory COVID‑19 vaccination policy and proof may be required prior to start date.

Responsibilities

  • Assess care needs with patients and families and determine service eligibility.
  • Link patients with service providers and coordinate care plan delivery.
  • Coordinate and monitor care plan delivery with a multidisciplinary team.
  • Establish a trusting relationship with patients and families.
  • Balance patient needs with available resources while respecting values and preferences.
