The salary range for this position is CAD $49.20 - $67.08 / hour.

Detailed Overview

In accordance with the BC College of Nurses and Midwives (BCCNM) standards and Fraser Health’s values, the Patient Care Coordinator provides clinical supervision, guidance, and support to nursing staff, other clinical team members, and support staff within the assigned units. They are responsible for daily operations, patient flow, and participate in research, policy, quality improvement, and risk management activities.

Responsibilities
  • Manage patient admissions, discharges, and referrals, ensuring optimal patient flow and care planning.
  • Supervise nursing and support staff, coordinate daily work, monitor care quality, and adjust staffing as needed.
  • Promote interprofessional collaboration, support staff development, and role model best practices.
  • Lead or support care conferences, disseminate clinical information, and support staff education and training.
  • Address patient and family concerns, participate in quality improvement initiatives, and collaborate on research and special projects.
  • Assist in staff recruitment, performance evaluations, and resource management.
  • Participate in committee work and develop evidence-informed policies.
Qualifications

Education and Experience: Current RN registration with BCCNM, three years of recent clinical experience, including one year of supervisory experience, and relevant postgraduate coursework or certification.

Skills and Abilities: Clinical expertise, leadership, staff coordination, communication skills, problem-solving, and physical ability to perform duties.
