Job summary

Interior Health Authority is seeking a relief part-time Patient Care Coordinator (PCC) at 100 Mile Hospital in British Columbia. The PCC will lead patient care coordination, support a multidisciplinary team, and ensure high-quality care for the community.

The role is a relief position with a structured schedule and contributes to patient flow and resource management. The PCC works rotating day shifts, collaborates with staff, and adheres to BCCNM standards and patient-centered care principles.

Qualifications

• Standards of practice and code of ethics per BC College of Nurses and Midwives (BCCNM)
• Ability to coordinate patient care and support a multidisciplinary team

Responsibilities

• Coordinate day-to-day operations and quality patient care within designated clinical area(s).
• Support the seamless movement of patients through the healthcare system and access to resources.
• Supervise and serve as a resource to staff for clinical decision-making and education of families.
• Review and make recommendations on administrative systems such as policies, procedures and standards of patient care, staffing levels, and quality improvement.
• Liaise with the interdisciplinary team for resource planning and staff educational needs.

Join the team at 100 Mile Hospital as a relief part-time Patient Care Coordinator (PCC). This is an exciting opportunity for a skilled and organized healthcare professional to provide leadership, coordinate patient care services, and support a multidisciplinary team in delivering exceptional care to the community. This relief position is available until October 17, 2027, or until the return of the incumbent.

The position works rotating day shifts, Tuesday to Friday, from 07:00 to 16:24.

What will you work on?

The Patient Care Coordinator (PCC) practices in accordance with the standards of professional practice and code of ethics as outlined by the British Columbia College of Nurses and Midwives (BCCNM) as well as within a patient and family centered care model. The PCC is responsible for the coordination of day-to-day operations and quality patient care within a designated clinical area(s). This position supports the seamless movement of patients through the healthcare system and in accessing appropriate resources. The PCC supervises as well as serves as a role model and a resource to staff for clinical decision-making, clinical assessments of patients and in the counseling and education of families. Reviews, analyses and makes recommendations on the development of administrative systems such as policies, procedures and standards of patient care, adequate staffing levels, and a continuous quality improvement environment. Liaises with the interdisciplinary team for identification, coordination and efficient utilization of resources as well as plans for staff educational needs.
