Duties

As an integral member of the interprofessional team, the site-based Patient Care Coordinator (PCC) acts as a resource and co-leader for the interprofessional disease site team(s). The PCC has clinical and leadership expertise. The principal responsibilities within the site(s) include:

1. Acting as a resource for the interprofessional disease site team(s).
2. Co-leading the interprofessional team.
3. Applying clinical and leadership expertise in patient care.
