Indwell in Simcoe, Ontario, seeks a PART-TIME Supportive Housing Coordinator to help tenants with mental health concerns maintain housing stability and develop personal service plans. The role involves collaborating with an interdisciplinary team, applying harm-reduction approaches, and coordinating services to promote health, wellness, and belonging.

The ideal candidate has case-management experience, PVSC clearance, First Aid/CPR, and a compassionate, independent work style.

Competenze

Experience within a case management framework with individuals with serious mental illness, substance use, or other needs.
PVSC current within Indwell requirements (at applicant's cost) and valid First Aid & CPR certification.
Current Standard First Aid & CPR Certification from an approved provider (or willingness to obtain) and a valid driver’s license.

Mansioni

Direct support to tenants using a recovery-oriented, trauma-informed approach.
Coordinate, lead and participate in case conferences with outside supports.
Develop and carry out individual resident support plans and assessment tools.
Connect residents with clinical and informal supports; facilitate case management.
Develop group programming and maintain tenant health records thoroughly and timely.
