Job summary

Hopewell Support Services is seeking a compassionate Support Services Worker for a Regular Part Time role in Guelph, Ontario. The position supports individuals with developmental and physical disabilities across multiple sites within Guelph-Wellington and works within a clinical multidisciplinary team.

The role involves implementing care/service plans, assisting with daily living activities, and helping individuals live as independently as possible.

Qualifications

• Experience in social or community services is preferred.
• Experience working with individuals with developmental disabilities is advantageous.

Responsibilities

• Support individuals to live independently and participate in meaningful activities.
• Assist with daily living tasks (toileting, grooming, meals) per care plan and routines.
• Follow Behavior Support Plans and document changes and progress with the team.
