Qualifications

• Current Certificate of Registration with the College of Nurses of Ontario (O. Reg. 246/22, s. 51).
• Experience in a Long-Term Care setting is an asset.
• Performs care planning and coordination of care within a diverse multidisciplinary care team.
• Demonstrates use of current care technology.
• Strong leadership, communication and interpersonal skills.
• Ensures MOH and MLTC requirements are met.
• Must demonstrate good communication skills.
• Must demonstrate an active interest in ongoing education, training, and professional development.
• Must be computer literate.

Responsibilities

• Deliver direct care in accordance with established care plans.
• Ensure accurate and timely documentation of resident information.
• Facilitate orientation of new team members and residents.
• Participate in assigned committees and quality improvement initiatives.
• Coordinate care with a diverse multidisciplinary team.
• Manage supplies and maintain the physical environment and equipment.
