About Perennial

Perennial embeds physician- and nurse-practitioner-led primary-care clinics directly inside retirement residences, bringing concierge medical and allied-health services to older adults where they live. Our interdisciplinary team (MDs, NPs, RNs, RMTs, OTs, dietitians) is united by one goal: keeping residents healthy, mobile and out of hospital. Learn more at perennialcare.ca.

About The Role

As a Perennial Physiotherapist you’ll bring hands-on, evidence-based care directly into our partner retirement residences. You’ll start by running one or two on-site clinic days per month in Hamilton and Oakville (4- to 8-hour blocks) and expand your schedule as the program becomes busier. There is also an opportunity to pick-up additional shifts at other locations if desired.

What You’ll Do
  • Perform comprehensive MSK and neurological assessments for seniors and residence staff.
  • Deliver manual therapy, therapeutic exercise, gait and balance training.
  • Design and lead individual or small-group fall-prevention and mobility programs.
  • Educate residents and caregivers on safe movement and home-exercise strategies.
  • Chart detailed SOAP notes and outcome measures.
  • Submit electronic insurance claims and collect any resident co-pays.
  • Communicate progress and red-flags to Perennial physicians, nurse practitioners, and allied-health peers.
  • Maintain a portable treatment kit and adhere to all IPAC standards.
Qualifications
  • Registered Physiotherapist in good standing with the College of Physiotherapists of Ontario.
  • Minimum two years’ experience in orthopaedics, geriatrics, or community rehabilitation.
  • Strong manual-therapy, exercise-prescription, and patient-education skills.
  • Valid Ontario driver’s licence and reliable transportation; able to travel between sites.
  • Assets: certification in fall-management, vestibular rehab, acupuncture/dry needling, or group-class leadership.
Why Join Perennial?
  • Flexible, low-commitment scheduling that grows alongside your existing practice.
  • Turn-key logistics: Perennial handles space, marketing, bookings.
  • Purpose-driven mission focused on keeping seniors mobile, independent, and out of hospital.
  • Collaborative interdisciplinary environment with physicians, NPs, RNs, RMTs, and other allied-health clinicians.
  • Clear growth path — expand hours, add new residences, or move into program-lead roles as demand scales.
