Job summary

Community Living Hamilton is seeking Direct Support Facilitators to provide supportive services in our group living settings. You will deliver social, recreational, and learning-based activities within the community, while ensuring the safety and dignity of clients.

The role requires a related degree or diploma (DSW/SSW/CYW or Social Sciences) with preferred DSW Certificate, plus 2 years with adults with developmental disabilities or related conditions.

Qualifications

  • DSW/SSW/CYW or Social Sciences degree/diploma in a related field; DSW Certificate preferred.
  • Two (2) years of recent experience with adults with developmental disability, transitional aged youth, addictions and/or mental health preferred.
  • Experience building rapport with clients with focus on respect, dignity, inclusion and boundaries.
  • Experience developing Person Directed Plans/ISPs with skill development, recreation, independent living skills, employment, health and wellness, and relationship building.
  • Ensure safety, security and wellness of clients, co-workers and visitors.
  • Demonstrates conduct in the community that enhances the reputation of clients and Community Living Hamilton.
  • Valid NVCI Certification and First Aid/CPR Certification.
  • A valid driver’s license.
  • Fluency in a 2nd language and additional training/ certifications are a plus.

Responsibilities

  • Provide support and activities for clients in group living settings.
  • Deliver social, recreational and learning-based activities within the community.
  • Maintain safety and well-being of clients, staff and visitors.
  • Build professional rapport with clients with emphasis on respect and inclusion.
  • Develop and implement Person Directed Plans/ISPs for client growth and independence.
  • Represent Community Living Hamilton in a positive manner in the community.

Overview

Direct Support Facilitators - Group Living provide support and assistance to clients within our group living settings, delivering social, recreational and learning based activities within the community.

Pay rate :29.28

Qualifications

  • DSW, SSW, CYW, or Social Sciences degree or diploma in a related field. The
  • above education with a DSW Certificate is preferred.
  • Two (2) years of recent experience working with adults with a developmental
  • disability, transitional aged youth, addictions and/or mental health preferred.
  • Experience building an effective professional rapport with clients by focusing
  • on respect, dignity, inclusion, and maintaining personal boundaries.
  • Experience developing Person Directed Plans/Individual Support Plans with
  • consideration for skill development, recreation, independent living skills,
  • employment, health and wellness, and relationship building.
  • Ensure the safety, security and wellness of clients, co-workers, and visitors.
  • Displays conduct in the community that enhances the reputation of clients
  • and Community Living Hamilton.
  • Valid NVCI Certification.
  • Valid First Aid/CPR Certification.
  • A valid driver’s license.

Additional consideration will be given to candidates possessing:

  • Fluency in a 2nd language
