Job summary

Extendicare is seeking a Part-time Dietitian at Winbourne Park, Ajax, Ontario. You will assess residents, plan nutrition and hydration care, and work with the interdisciplinary team to optimize outcomes.

The role involves reviewing menus and educating residents, families and staff on nutrition strategies. Ideal candidates are Registered Dietitians eligible for the Ontario College, with geriatric nutrition knowledge and strong communication, organization, and teamwork skills.

Qualifications

• Registered Dietitian eligible for membership with the College of Dietitians of Ontario.
• 3–5 years experience in long-term care preferred.
• Excellent oral and written communication skills.

Responsibilities

• Perform nutritional assessments using RAI MDS 2.0 as required and create care plans.
• Determine individualized diet plans with resident, family, and care team input.
• Develop, implement and evaluate nutrition programs and monitor goals.
• Educate residents, families and staff on nutrition and hydration recommendations.
• Collaborate on interdisciplinary teams and participate in committees.
