Job summary

Extendicare is seeking a Part-time Dietitian to support nutrition and hydration programs for residents. You will assess residents, create care plans, and collaborate with the Dietary Manager and Director of Care to meet individual nutritional needs.

You will review seasonal menus, educate residents and families, and participate in interdisciplinary teams ensuring quality indicators are met. A strong background in geriatric nutrition and RAI MDS 2.0 is preferred.

Qualifications

• Registered Dietitian, eligible for membership with the College of Dietitians of Ontario.
• Extensive knowledge of geriatric nutrition; 3-5 years experience in long-term care preferred.
• Proficiency with computer applications (Microsoft Office and internet).
• Experience with RAI MDS 2.0 desirable; familiarity with PointClickCare an asset.
• Excellent organizational, time management and multitasking skills.
• Knowledge of Occupational Health & Safety practices, principles and legislation.

Responsibilities

• Assess residents' nutritional needs and develop individualized care plans.
• Consult with Dietary Manager and Director of Care on nutrition and hydration needs.
• Implement and follow up on nutritional plans for residents.
• Develop and evaluate nutritional care plans, identifying concerns, goals and interventions.
• Maintain communication with the Dietary Manager about changing resident needs.
• Review seasonal menus for nutritional adequacy and provide written approvals.
• Educate residents, families and care team on nutrition and hydration recommendations.
• Participate in interdisciplinary teams and committees related to resident care and quality indicators.
