Saskatchewan Health Authority is seeking a part-time temporary Assessor Coordinator (CS/CPAS) in Saskatoon. The role focuses on case management, assessment, planning and coordination of community services within SHA Saskatoon (CPAS - Community C).

Preference is given to HSAS members under the SAHO/HSAS Collective Agreement. Education required includes a Bachelor of Social Work (BSW); a valid Class 5 driver’s license is also required.

Qualifications

• Bachelor of Social Work (BSW) required.
• Valid Class 5 driver's license.
• Registered/eligible for registration with the appropriate licensing body.
• Preference for HSAS members as per collective agreement.

Responsabilités

• Provide case management services including assessment, planning, monitoring and evaluation.
• Coordinate client/patient access services for SHA Saskatoon across community-based programs.
• Serve as entry access and ongoing coordinator for a variety of community-based services in Saskatoon.
