Job summary

The Hospice of Windsor and Essex County is seeking a Registered Practical Nurse to provide direct, patient-centered end-of-life care within our Hospice Residence. You will assess needs, implement care plans, and collaborate with an interdisciplinary team to promote comfort, dignity, and quality of life for residents and families.

As part of our front-line staff, you will manage medications, participate in symptom management, and contribute to a supportive work culture.

Qualifications

• Legally entitled to work in Canada.
• French language skills are an asset.
• Flexible hours including days, nights, weekends, and holidays in rotation.

Responsibilities

• Provide direct, hands-on care including medication administration and wound care.
• Develop, implement and evaluate individualized care plans aligned with patients' goals.
• Collaborate with physicians, social workers and nurses to ensure holistic, patient-centered care.
• Maintain documentation in accordance with professional standards and organizational policies.
• Support orientation and mentorship for new staff.
