Qualifikationen

• Registered Nurse (RN) in good standing with regulatory body
• 1+ year of recent community health experience
• Palliative care training/certification (e.g., Fundamentals, LEAP, CAPCE)

• Assess care needs with patients and families; determine service eligibility and develop care plans
• Link patients with community providers to prevent hospital admissions or ED visits
• Coordinate and monitor delivery of care plans
• Establish trusting relationships with patients and families
• Serve as subject matter expert on palliative community care
