Job summary

Pacific Coast Community Resources is seeking a Community Support Worker for the Awake Overnight shift in Port Alberni. You will work in staffed homes within the community, supporting individuals to lead independent lives while upholding high standards of care.

Qualifications

Strong interpersonal and communication skills required to interact with clients, families, and professionals.
Ability to document accurately and maintain case notes in electronic systems.
Basic computer literacy to operate data entry and documentation software.

Responsibilities

Advocate for the rights and choices of supported persons.
Interact effectively with people we support to promote independence.
Empower clients to be full participants in their community.
Develop, implement, and document Personal Service Plans.
Foster relationships with families, professionals, and community members.
Assist with activities of daily living and medication administration.
Apply behaviour management principles and mental health awareness in daily care.
Record daily activity and personal health information in the case management system.
Maintain a clean, safe home environment including light housekeeping and meal prep.
