Job summary

CBI Home Health in Sarnia-Lambton invites an Occupational Therapist to join our community-focused team. You will assess clients at home, plan interventions, and coordinate care with a multidisciplinary team.

This role emphasizes compassionate, person-centered care and the ability to improve daily living activities in home settings. You must be registered with the College of Occupational Therapists of Ontario and able to travel within the region.

Qualifications

• Registered with the College of Occupational Therapists of Ontario in good standing.
• Ability to travel within the community; valid driver's license and insurance.

Responsibilities

• Assessment & Implementation: Provide OT interventions, including ADL/IADL strategies, equipment recommendations, and home modifications.
• Team Collaboration: Use a collaborative, interdisciplinary approach to care and services.
• Holistic Care: Support a restorative approach to health, offering interventions, health coaching, and self-management support.
• Digital Health: Utilize our digital platforms for care delivery, collaboration, documentation, and intervention.

Salary Range: The salary range for this position is $82 - $86 per visit
