Job summary

Lifemark Health Group in Wallaceburg, ON is seeking a compassionate Occupational Therapist to join our largest rehabilitation network. You will provide evidence-based OT services, conduct admissions and ADL assessments, and recommend equipment solutions to improve function and safety in seniors.

You’ll collaborate with an interdisciplinary team, document care plans, and support ongoing learning through our mentorship and development programs.

Qualifications

• Registered and in good standing with the College of Occupational Therapists in your province
• 1-3 years of experience as an Occupational Therapist
• Experience in Long-Term Care is an asset
• Excellent written and verbal communication skills
• Valid Driver's License may be required
• Valid Vulnerable Sector screening check
• Tuberculin clearance (2-Step Mantoux) within 6 months

Responsibilities

• Provide comprehensive, evidence-based Occupational Therapy services to clients
• Perform clinical evaluations, including admission assessments and ADL assessments
• Identify equipment needs and initiate recommendations
• Collaborate with vendors for equipment trials and funding applications
• Educate on seating, daily living activities, and assistive devices to improve function
• Evaluate restraint use, wound prevention, falls and injury prevention
• Document assessments and interventions in client charts
• Excel in a collaborative, inter-disciplinary team environment
• Exhibit sound professional judgment

Schedule: 2 hours/week

Your impact

• Provide comprehensive, evidence-based Occupational Therapy services to clients
• Perform clinical evaluations, including admission assessments, ADL assessments, specialized seating and positioning assessments
• Identification of appropriate equipment needs with initiation of recommendations
• Consult with local vendors regarding equipment trials and complete applications/funding letters to Ministry of Health, insurance agencies and other funding parties
• Provide education on seating and positioning, independence with activities of daily living, wheelchairs and adaptive aides as well as providing ongoing education to clients and family to improve function, independence and safety
• Provide ongoing evaluation of restraint use, wound prevention, falls, and injury prevention.
• Record assessment and treatment interventions in the client's chart
• Excel in a collaborative, inter-disciplinary team environment
• Exhibit sound professional judgment

As one of the largest networks of OTs in the nation, you will work within a supportive, multidisciplinary environment, delivering high-quality, client-centered care across multiple long-term care, or specific long-term care homes. Key skills and qualifications include:

• Registered and in good standing with the College of Occupational Therapists in your province.
• 1-3 years of experience as an Occupational Therapist (New Grads are welcome to apply)
• Experience within a Long-Term care will be an asset
• Excellent written and verbal communication skills.
• Knowledge of current Occupational Therapy standards, modalities and scope of practice; or an equivalent combination of education, training and experience.
• Valid Driver's License and reliable vehicle may be required
• Valid Vulnerable Sector screening check
• Proof of Tuberculin (2-Step Mantoux Test) clearance (within 6 months)

• Superior interpersonal relations, communication, problem-solving, time management, computer and documentation skills
• Proficiency in computer skills
• Demonstrated ability to work collaboratively as a member of a multidisciplinary team
• ADP Authorized Certification
