Job summary

CarePartners is seeking an Occupational Therapist to join our rehabilitation team in Stratford, Ontario, with flexible scheduling and self-scheduling model. We offer full-time, part-time, and casual roles across Stratford and surrounding areas.

The OT will assess patients in home and community settings, develop care plans, and collaborate with the Ontario Health atHome program. A Master’s degree and OT registration are required, with travel as needed.

Qualifications

  • Master’s Degree in Occupational Therapy from a recognized university.
  • Registration with the College of Occupational Therapists of Ontario.
  • Proven computer skills, including email, MS Word and Outlook, work scheduling systems.
  • Strong assessment skills and the ability to participate in effective treatment programs.
  • Provide one-on-one therapeutic support with clients.
  • Promotes independence of the client in the home and in the community.
  • A reliable vehicle for some regular travel.
  • Excellent English verbal and written communication skills.
  • French language skills are an asset.
  • Clear Background and Vulnerable Sector Check.

Responsibilities

  • Provides coordination, supervision, and coaching to the Community Support Workers in the field.
  • Professional assessment of patients in the community and in their homes.
  • Develops and maintains an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
  • Works with the patient on appropriate time-specific, measurable goals and outcomes in collaboration with the care team.
  • Works independently and as an integral member of an interdisciplinary care team.
  • Provides active guidance for workplace Health and Safety.
  • Performs other duties as required.

We are currently looking for someone to join our dynamic Rehabilitation Team in the role of Occupational therapist. We are actively expanding our therapy/rehabilitation program and are looking for professionals who want to be a part of this exciting growth.

We are hiring for Full-time, Part-time and Casual positions in the Stratford, ON and surrounding areas.

  • This position offers a competitive compensation grid, with progression based on hours worked and role-specific criteria. The OT per visit range is $74.48 to $86.85and the hourly range is $37.23 to $43.41. Placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.

What The Role Involves
