Job summary

Qualifications

  • Master's Degree in Occupational Therapy or recognized equivalent.
  • Registered and in good standing with the College of Occupational Therapists of Ontario (COTO).
  • Experience supporting older adults and facilitating safe transitions home is preferred.
  • Proficiency with electronic documentation systems and secure digital communication.

Responsibilities

  • Complete comprehensive occupational therapy assessments and develop individualized treatment plans.
  • Provide ongoing evaluation and modification of treatment plans based on client progress.
  • Support clients transitioning from hospital to home through evidence-based interventions.
  • Communicate assessment findings and education to clients, families, and care partners.
  • Maintain accurate clinical documentation in accordance with professional standards.
