Job summary

Bayshore Integrated Care Solutions (ICS) in Ontario is seeking an Occupational Therapist to join an integrated care team. You will assess, treat, educate and manage clients transitioning from hospital to home, helping older adults maximize independence and safety.

You will work within one interdisciplinary team, communicate findings to clients and families, maintain timely documentation, and participate in case conferences.

Qualifications

• Master’s Degree in Occupational Therapy
• Registered with the College of Occupational Therapists of Ontario (COTO)

Responsibilities

• Perform comprehensive occupational therapy assessments and develop individualized treatment plans and discharge recommendations.
• Conduct ongoing evaluation and modify plans based on client progress.
• Support clients transitioning from hospital to home with interventions that promote safety and independence.
• Collaborate with interdisciplinary team to ensure coordinated care.
• Communicate findings and education to clients, families and partners.
• Maintain accurate clinical documentation per provincial standards.
• Provide guidance to support personnel as appropriate.
• Participate in case conferences and care planning.

As an Occupational Therapist withICS, you'll be part of an integrated care model where collaboration, visibility, and continuity of care are at the center of everything we do. Working within one interdisciplinary team and one shared client record, you'll have visibility into the entire client journey and the opportunity to see the impact of your interventions throughout recovery. You'll be responsible for the assessment, treatment, education, and ongoing management of clients requiring Occupational Therapy services, helping older adults maximize independence, safely transition home, and achieve meaningful outcomes in their communities.

Clinical Responsibilities

• Complete comprehensive occupational therapy assessments and develop individualized treatment plans, goals, and discharge recommendations.
• Conduct ongoing evaluation and modification of treatment plans based on client progress and changing needs.
• Support clients transitioning from hospital to home through evidence-based interventions that promote safety, independence, and quality of life.
• Collaborate with interdisciplinary team members to ensure coordinated and seamless care delivery.
• Communicate assessment findings, recommendations, and education to clients, families, caregivers, and healthcare partners.
• Maintain accurate and timely clinical documentation in accordance with the standards of the College of Occupational Therapists of Ontario.
• Provide clinical guidance and direction to support personnel involved in the delivery of Occupational Therapy services as appropriate.
• Participate in case conferences, care planning discussions, and collaborative decision-making processes.

Education & Registration

• Master’s Degree in Occupational Therapy or recognized equivalent.
• Registered and in good standing with the College of Occupational Therapists of Ontario (COTO).
