Bayshore HealthCare is seeking an Occupational Therapist to join our integrated care teams in Ontario. You will perform comprehensive OT assessments, design individualized plans, and facilitate safe transitions from hospital to home, working with nurses, PTs, social workers, and other clinicians.
You will document progress, adjust care plans as needed, and contribute to proactive, patient-and-family-centered care within a nationally recognized home health organization.
